David Upton’s thoughtful and comprehensive discussion of national insurance for mental health care is timely and everywhere to the point. Even where one might not agree, it must be admitted that the issues raised are valid and important for public debate and informed decision. If we may not achieve an ideal system, let us at least try to learn from our own previous mistakes and to avoid those that have been made in other countries. This commentary will focus on two major concerns—care for the elderly, and care for psychotic (mentally ill) patients.
