Evolution of Renal Osteodystrophy in Patients Treated by Continuous Ambulatory Peritoneal Dialysis

Abstract

To remove the excess of phosphate and correct the deficit of calcium represent the most important goals to be achieved in uremic patients treated by maintenance dialysis methods in order to prevent the multiple complications related to hypersecretion of parathyroid hormone whose stimulation is enhanced by hypocalcemia and hyperphosphatemia (1,2). Despite a general agreement on this statement and ongoing progress in hemodialysis techniques and strategies, this aim remains by far not successfully reached in all patients treated by chronic hemodialysis, whose long survival is all too often complicated by severely disabling bone disease (3). The very principle on which the technique of Continuous Ambulatory Peritoneal Dialysis (CAPD) is based seemed likely to provide a more satisfying solution than hemodialysis for coping with the problem (4,5). A slow but continuous removal of phosphate (Pi) together with a simultaneous supply of calcium (Ca) through the peritoneal route, as well as the “steady” metabolic state resembling the one of a normal subject could prove as more efficient for maintaining a satisfactory calcium-phosphate balance than the “permanently unstable” situation which is the one of the patients treated with maintenance hemodialysis.
