Clinical Applications in the Area of Contraceptive Development

“Leads” for new contraceptive development include possible direct manipulation of ovarian function and are an outgrowth of studies on basic gonadal physiology. Being able to perturb normal reproductive activity by altering some aspect of ovarian function also may provide evidence for a specific role for that function. The research thus serves the dual role of advancing basic knowledge while providing possible clues for new contraceptive modalities and human application. Present contraceptive methodology ranges from high efficacy with low (but marginally acceptable) risks of adverse effects as with the pill to the less effective techniques that carry the inherent risk of pregnancy when they fail as for most barrier techniques. With an effectiveness of 99% and a mortality risk one fifth that of a pregnancy, the pill is still a hard act to follow. It is unlikely that drug regulatory agencies will allow marketing of any new product that clearly does not surpass present methods. Contraceptive development involving the ovary clearly is not yet in so favorable a category.
