Trimethoprim/Sulfamethoxazole Synergy and Prostatis

TMP/SMX is often effective in the treatment of bacterial prostatitis, but, because of the concentration of TMP in the prostatic fluid of dogs is four times that in plasma and the concentration of SMX in this fluid is only one-tenty of that in plasma, the question arises as to whether SMX contributes to the efficacy.

The ratio of TMP to SMX in plasma of man is around 1:20 but, based on observations in dogs, it is only 2:1 in the prostatic fluid. Although synergy between TMP and SMX occurs over a wide range of ratios, the 2:1 ratio is strikingly different from the 1:20 in the plasma which, based on maximum reduction in MIC, is near optimal for the majority of pathogenic bacteria. Therefore, the MIC of TMP and SMX, singly and together, in the ratios of 2:1, 1:1 and 1:2 have been determined for 24 strains of bacterial species likely to cause prostatitis. Four strains were sulfonamide-resistant and these were the only ones which did not show synergy with the combinations.
