The Diabetic Pancreas

Abstract

Early investigations of the effects of pancreatic transplantation1,2 combined with results obtained from pancreatectomy studies3 provided definitive evidence for the involvement of the pancreas in diabetes mellitus. The hypothesis of the endocrine nature of the islets of Langerhans4 was confirmed by subsequent duct-ligation experiments demonstrating the role of the islet tissue component in diabetes mellitus.5,6 The discovery and extraction of insulin and its clinical application in the successful control of diabetes7 for a time lessened interest in pancreatic transplantation. However, the subsequent realization that insulin therapy alone was apparently insufficient to completely reverse the consequences of diabetes—the associated vascular lesions—generated renewed interest. As with other organs, immunological barriers complicated studies on the feasibility of pancreatic transplantation. This problem could be partially or even completely circumvented in the laboratory by immunosuppression, irradiation, autotransplantation, or the use of immunologically privileged sites or highly inbred strains of experimental animals. However, pancreatic transplantation has been, in addition, further complicated by the morphological nature of the mammalian pancreas itself. With the islet tissue distributed throughout the exocrine organ as small islands8 comprising only 1–2% of the gland,9–14 the question of how best to provide the diabetic recipient with a new complement of functioning islet tissue has been approached from two aspects.
