Nephropathy Induced by Heavy Metals

Occupational and/or environmental chronic exposure to heavy metals implies a high nephrotoxic risk. This activity of metals can result in both acute and chronic syndromes concerning particularly glomerular and/or tubulointerstitial structures.

Chronic tubulointerstitial nephropathies are almost exclusively caused by lead and cadmium exposure.

Lead nephropathy has an insidious and progressive trend and diagnosis is difficult because both clinical and histologic signs are non-specific and variable. Problems related to the relationship between lead exposure and hyperuricemia, gout and hypertension and diagnosis problems in lead nephropathy are discussed, also on the basis of clinical cases kept under our observation.

Manifestations of cadmium nephropathy are various and glomerular and tubulointerstitial structures are simultaneously involved. Clinical and epidemiologic aspects are dealt with on the basis of data reported in literature.

Finally, problems concerning biological monitoring in exposed workers are discussed.
