Abstract
This chapter reviews the major gross and microscopic features of ulcerative colitis and Crohn’s disease, with an emphasis on the earliest histologic features of these conditions and their distinction from acute self-limited colitis and artifactual changes, especially those encountered in biopsy material. In addition, particular attention is paid to features that can be considered “atypical” in patients with ulcerative colitis, namely rectal sparing and patchy involvement of the bowel mucosa, “backwash ileitis,” involvement of the upper GI tract, peri-appendiceal inflammation, fulminant and indeterminate colitis and pouchitis.
