Abstract
Laparoscopic instrumentation outside the peritoneal cavity has evolved as a means of addressing the extraperitoneal organs while avoiding potential complications inferred from entering the peritoneum. Standard laparoscopic techniques are equally applicable both within and outside the peritoneal cavity, but the generation of pneumoperitoneum and exposure to ensure adequate surgical view may be particularly challenging. In this chapter, we describe the anatomical challenges associated with extraperitoneal access and outline the initial laparoscopic approach to extraperitoneal structures.
