Abstract
Since cis-platinum was introduced as a chemotherapeutic agent, there have been many pharmacologic manipulations utilized to improve its therapeutic index. Initially, efforts were directed toward reducing toxicity. Dose-limiting nephrotoxicity has been successfully ameliorated by a variety of hydration schema including mannitol diuresis, sodium thio- sulfate administration and the use of hypertonic saline.(1, 2, 3) Severe nausea and vomiting have been avoided using metoclopramide and other ant i-emitics.(4) Second generation anologues with less nephrotoxicity are now being studied.(5) These efforts have succeeded in improving the therapeutic index for cis-platinum to some degree by reducing toxicity and thereby increasing the maximum tolerated dose.
