Abstract

Quantitative assessment of bone mineral is necessary for an understanding of those factors which influence the development of osteoporosis and the investigation of possible remedial measures. Most studies have been upon groups, rather than individuals, and substantial overlap between osteoporotic and normal subjects and between different disease and treatment groups has been shown, in spite of significant differences of mean values. However, it is now being argued that there is a case for screening individuals to identity subjects with a bone mass low enough to suggest a high risk of vertebral or femoral fracture, so that prophylactic measures may be recommended. Accuracy is clearly important for all measurements, especially when comparisons between different centres are used; for longitudinal studies precision is paramount. Techniques must also carry a negligible risk, from radiation for example. The most appropriate part of the skeleton for a particular purpose must be investigated. These factors will all be considered here.
