Abstract

Fecal bile acid excretion represents an important pathway for the elimination of cholesterol from the body. Extensive transformation of primary bile acids occurs due to the action of the intestinal microflora [1] resulting in a complex array of metabolic products being formed within the lumen which are subsequently excreted. Since the enterohepatic circulation of bile acids is extremely efficient, most of the bile acid pool is conserved by intestinal reabsorption; however, a small percentage of the bile acids escapes the normal reabsorption mechanism and is therefore excreted. In the steady state this fecal loss will be approximately equal to the daily synthesis, and therefore, accurate quantitative determination of fecal bile acid excretion provides important information about bile acid kinetics and cholesterol homeostasis. Qualitative analysis, on the other hand, provides insight into intraluminal events involving bacteria and bile acid interaction, since microbial activity will largely influence the pattern of fecal bile acid excretion. Diet, drugs, gut transit time, intraluminal environment, gallbladder emptying, and hepatic function are other factors that will have a bearing on fecal bile acid excretion, and these are discussed in some detail.
