Abstract

Much has changed in the clinical laboratory. Until 1985, there was attention paid to hospital census and the role of inpatient service in the provision of health care. Naturally, the clinical laboratory contributed much, usually as a “cash cow.” Since 1985, there has been a growing dependence on ambulatory services. Not only have hospital outpatient clinics grown but there has been an expansion of day treatment and day surgery. Because of the fiscal necessity for shortened inpatient stays, a substantial part of the health care dollar has been taken up by extended care facilities. The clinical laboratory, then, is competing for a smaller share of the available resources.
