Management of risk factors and other diseases in candidates for microneurosurgical anastomosis in cerebral ischemia

Abstract

Transient ischemic attack and, ultimately, completed brain infarction are associated with risk factors (Table 10.1) that occur singly or in combination in the majority of cases.(3) It is the purpose of this paper to examine such risk factors and to propose a set of categories into which patients bearing those risks ought to be grouped. This may help the clinician in selection of patients for surgery and also may provide a classification which would be incorporated into a randomized study of the efficacy of microneurosurgical anastomosis for cerebral ischemia. Atherosclerosis is the most ubiquitous of these risk factors and involves intracranial as well as extracranial arteries(2). It is present in 90% of patients with occluding cerebrovascular disease, and is associated with a series of collateral risk factors (Table 10.2) which should be considered as well. Cerebral embolism accounts for 8% of completed brain infarctions.
