Abstract

Only rarely have I encountered a patient whose large breasts are amenable to reduction by circumareolar mastopexy. I usually feel that the excessive breast enlargement makes it difficult to obtain the double advantage of relatively satisfactory breast reduction associated with good shape and an acceptable scar. I would be tempted to reserve the circumareolar technique in large breast reduction to the patient who happens to have very loose skin. This allows her breasts to be reduced by liposuction more than by actual surgical reduction, thus easing the actual reduction and imposing less taxing demands on the tissues of the skin to hold the breast mound.
