Abstract
There may be some truth to the idea that a traditional health care system, centered in institutions and bent on precise diagnosis and cure, is spinning its expensive wheels in attempting to solve the problems of some complicated dependent old people. Often these patients neither want nor can benefit from hospital-style care. If the effort and time now sometimes wasted in trying to convince them to accept it anyway were spent improving their custodial care at home, might we all be further ahead?
