Abstract
In this chapter, we will uncover the difficult problem of organizational influence on HCD, and potentially the influence of HCD on organizations. Many authors have already investigated and analyzed such problem. In the human error arena, Reason (Human error, 1990) provided the famous Swiss cheese model. This model provides a macroscopic view of what it requires to prevent potential accidents by developing appropriate defenses. However, it does not tell us how the organization should be designed to manage socio-technical reliability. Kletz (2000) addressed the influence of senior management on functional risk, and its involvement in the detail of safety issues with close attention to output, costs, and even product quality.
