Abstract
A 58-year-old man with a background history of ischemic heart disease and previous coronary bypass grafting presented with several episodes of heart failure. His echocardiogram showed severe mitral regurgitation with a broad jet secondary bileaflet restriction. He underwent edge-to-edge repair with the MitraClip system. An initial MitraClip was inserted, but the patient required a second clip, to further reduce the severity of the regurgitation, and the optimal position of the second clip was determined by balancing risk of reducing mitral regurgitation at the cost of producing mitral stenosis.
