Abstract

Surgical control of pathological reflux entails:

  1. 1.

    Mobilisation and reduction of the associated hiatus hernia.

  2. 2.

    Repair of the hiatus by approximating the crura.

  3. 3.

    The performance of an anti-reflux procedure to buttress the weak lower oesophageal sphincter. For this final step the choice of procedure will vary. However, the initial steps are common to all.

  4. 4.

    If reduction of the hernia cannot be accomplished without tension on the repair, an oesophageal lengthening procedure should be interposed between steps 2 and 3.
