Abstract

The development of radiotherapy in the management of brain tumours has been largely empirical, but since its introduction it has developed into one of the most effective treatment modalities in the management of patients with intracranial tumours. In some rare malignant brain tumours such as germinoma, radiotherapy is curative. The excellent disease control following conservative surgery and radiotherapy in the more benign tumours such as optic nerve glioma, pituitary adenoma and craniopharyngioma has established radiotherapy as the essential component of treatment. In highgrade gliomas, the commonest of malignant brain tumours, radiotherapy achieves prolongation of disease-free survival and survival. The value of radiotherapy has been demonstrated in prospective randomized studies in high-grade gliomas. In other brain tumours the role of radiotherapy is not proven beyond doubt and we have to rely for proof of its effectiveness on selected series of patients referred for radiotherapy largely following incomplete tumour excision.
