Preoperative Evaluation of Chronic Venous Insufficiency with Duplex Scanning and Venography

Abstract

Preoperative evaluation of patients who suffer from chronic venous disease due to previous deep venous thrombosis or primary valvular incompetence has substantially changed in the last decade. Color duplex scanning is now widely available, and it has become the most useful preoperative test for evaluation of chronic venous insufficiency. We reserve contrast venography for those patients who have evidence of significant deep venous occlusion or for those who are potential candidates for deep vein reconstructions. In this chapter we will discuss advantages of duplex scanning over other diagnostic tests and present techniques of duplex examination we use to confirm valvular incompetence in leg veins. We also discuss current indications and technical details of ascending and descending contrast venography.
