Before discussing the pitfalls of actual induction of anaesthesia, certain preliminary procedures deserve attention. A routine must be established to prevent anaesthetising the wrong patient or performing of the wrong operation. Although anaesthetists are not solely responsible they are part of a series of checking procedures, which includes nurses and surgeons, designed to ensure that errors do not occur.
