Reconstructive Surgery in Neuropathic Bladder

Abstract

The lower urinary tract consists of 2 components: bladder and urethra, both of which participate in the two phases of micturition. During the storage phase, the bladder acts as a large-capacity, low-pressure reservoir and outlet sphincteric mechanisms provide outflow pressure greater than intravesical pressure. During the voiding phase, bladder smooth muscle (detrusor) generates well-sustained and sufficient pressure in coordination with sphincteric mechanisms, which synergistically relax to lower outflow resistance. If lower urinary tract components function well, normal micturition takes place with protection of the upper urinary tract (UUT). The individual is continent and empties his or her bladder to its completion. If these components do not function appropriately, different types of voiding dysfunction may occur, and if a neurological pathology is the cause, this is called neuropathic bladder, or, more appropriately, neurogenic voiding dysfunction.
