Abstract
You are the only physician on a Boy Scout training sailboat, 2 days from the closest hospital. One of the Scouts stepped on a sharp, serrated fishhook which implanted itself deep in the mid plantar area of the right foot. You must excise the fishhook immediately to prevent infection. You have various syringes and needles along with one 20 mL bottle of 1% plain lidocaine in your medicine cabinet; what techniques could you use to eliminate pain during the painful excision of this fishhook?
