Abstract
The sinking of the Herald of Free Enterprise, the Piper Alpha oil-rig disaster, the King’s Cross fire, the M1 plane crash, the train crashes at Purley, Glasgow and Clapham, and the disaster at Hillsborough — events like these, caused by accident or neglect rather than by sabotage — have been all too frequent in the last few years. They are always followed by demands for an inquiry into causes and for the identification of anyone responsible for the failure of health and safety precautions.
