A 62-year-old man presented with cellulitis. He was found to have pancytopenia with 0% monocytes in the peripheral blood. Physical examination revealed splenomegaly, but no hepatomegaly or lymphadenopathy. The peripheral blood smear showed a few atypical lymphoid cells (Fig. 34.1). A bone marrow aspirate was obtained after two dry taps (Fig. 34.2). The core biopsy revealed patchy distribution of lymphoid aggregates (Fig. 34.3).
