Abstract

Dr. Barbara James sat down at her desk at her private family medicine practice to finish documenting on her last patient for the afternoon. It was a warm fall day in her small town in North Dakota and the day was busy. Her last patient was a 48 year old man who had walked in unscheduled with a large laceration on his left arm from an accident at the farm where he was working. He was not a regular patient and had no regular physician or insurance. Dr. James spent the better part of an hour stitching the laceration, providing needed immunizations, and just checking on his overall health status. He had no insurance and little income, but it was an opportunity to help. Dr. James was used to receiving payment in many forms – insurance, produce from the farmers, and work provided at her home in exchange for care of a family’s children – but also knew that she, along with the other primary care physicians in town, provided care for the uninsured in their community out of kindness.
