Das Gesundheitswesen in Deutschland ist fachlich hoch entwickelt und entsprechend differenziert. Für die Behandlung depressiver Patienten stehen je nach Art, Schwere oder Chronizität unterschiedliche Behandler zur Verfügung, mit unterschiedlichen Behandlungsangeboten, unterschiedlichen Ausstattungen, unterschiedlichen therapeutischen Interventionsmöglichkeiten, unterschiedlichen Kosten und unterschiedlichen Anforderungen an den Patienten. Im Einzelfall stellt sich die Frage, wann ein Patient wo optimal, aber auch mit dem geringsten Aufwand behandelt werden kann. Als Ordnungsrahmen bietet sich ein Stufenmodell der Versorgung (Helmchen und Linden 2000) an, das in Tabelle 23.1 zusammenfassend dargestellt ist.
