Zusammenfassung
Ein epileptischer Anfall ist durch eine episodische Funktionsstörung von Nervenzellen charakterisiert, die durch pathologische exzessive neuronale Entladungen infolge gesteigerter Erregbarkeit oder gestörter inhibitorischer Faktoren zustande kommt. Die klinischen Ausfälle hängen davon ab, welche Funktion die beteiligten Nervenzellen normalerweise besitzen. Prinzipiell unterscheidet man zwischen unprovozierten und provozierten Anfällen, sog. Gelegenheitsanfällen. Gelegenheitsanfälle werden ausschließlich durch exogene oder endogene Noxen ausgelöst und treten nicht unabhängig von diesem Auslöser auf. Das isolierte Auftreten eines epileptischen Anfalls rechtfertigt nicht die Diagnose eines Anfallsleidens. Eine Epilepsie liegt erst vor, wenn 2 oder mehr unprovozierte Anfälle im Abstand von mindestens 24 h aufgetreten sind.
Epilepsien bzw. epileptische Syndrome sind Erkrankungen unterschiedlicher Ätiologie, die klinisch durch wiederholtes Auftreten epileptischer Anfälle ohne erkennbare Auslöser charakterisiert sind oder bei denen diese ein führendes Symptom darstellen.
