Zusammenfassung
Zwei Monate vor Erstvorstellung in einer hämatologisch-onkologischen Ambulanz erkrankt ein 14-jähriger Junge kurzzeitig an einer febrilen, wässrigen Enteritis, die zeitgleich im Umfeld auftritt. Vier Wochen spä ter fallen beim fieberfreien Patienten erstmals blutige, schleimige Stühle auf, die mit schweren kolikartigen Bauchschmerzen einhergehen. Durch den Hausarzt erfolgt eine Behandlung mit Steroiden und Mesalazin. Im Blutbild fä llt eine Thrombozytenzahl von über 1000/nl auf. Serologisch findet sich kein Hinweis auf eine Infektion durch Campylobacter, Yersinien, Salmonellen oder Shigellen. Nachfolgend wird eine chronisch-entzündliche Darmerkrankung ausgeschlossen. Bei einer Thrombozytenzahl von 1320/nl und einer Hyperkaliämie (5,5 mval/l) wird der Patient mit Acetylsalicylsäure behandelt.
Zum Zeitpunkt der Überweisung ist der Patient subjektiv beschwerdefrei und der klinische Untersuchungsbefund un auffällig. Die Thrombozytenzahl beträgt zu diesem Zeitpunkt 3048/nl. Gleichzeitig besteht eine Leukozytose mit einer Leukozytenzahl von 17,5/nl bei normaler Differenzierung. Da keine Ursache für eine reaktive Thrombozytose gefunden werden kann, wird bei Persistenz der hämatologischen Auffälligkeiten eine Knochenmark-, Hä mostase- und Immundiagnostik durchgeführt und schließlich eine essenzielle Thrombozytose mit erworbenem Von-Willebrand Syndrom diagnostiziert.
