Zusammenfassung
Mit einem Anteil von 10–15% der viszeralchirurgischen Operationen sind Hernienoperationen heute ein wichtiger Kostenfaktor im Gesundheitssystem. In den letzten hundert Jahren hat sich die chirurgische Therapie der mit 80% am häufigsten vertretenen Leistenhernien kontinuierlich weiterentwickelt: Die konventionelle OP-Technik wurde verbessert und die Implantation von Netzen entwickelt bis hin zu den heute immer stärker verbreiteten laparoskopischen Verfahren.
Ein Vergleich der Operationsarten unter dem reinen Kostenaspekt zeigt, dass das konventionelle Verfahren bezogen auf die unmittelbaren Operationskosten das preiswertere ist. Jedoch entstehen hier Folgekosten durch längere Krankschreibung von Arbeitnehmern sowie eine höhere Rezidivrate und eine höhere Rate an chronischen Schmerzen, so dass für die Volkswirtschaft und langfristig für das Gesundheitssystem das laparoskopische Verfahren das kostengünstigere ist.
