Conclusion
Le traitement des gonarthroses fémoro-tibiales par ostéotomie reste actuellement une solution thérapeutique chirurgicale très utilisée dans l’attente du remplacement prothétique. Les bases théoriques de ces interventions sont de mieux en mieux connues. L’analyse radiographique, la planification préopératoire et la technique chirurgicale ont fait d’importants progrès. Dans l’arthrose interne, la plus fréquente, les résultats à court terme des ostéotomies tibiales de valgisation sont très bons, moyennant une correction en valgus de 3° à 6°. Cependant,les résultats à long terme de cette chirurgie sont inconstants et le pourcentage de corrections finales insuffisantes ou, au contraire, d’hypercorrections reste encore très élevé. Des progrès dans l’adéquation entre planification préopératoire et réalisation finale doivent être faits pour améliorer nos résultats à long terme. Les progrès de l’imagerie et l’apport de la chirurgie assistée par ordinateur constituent certainement des voies de recherche intéressantes pour l’avenir.
