Summary
The ELD flap is a useful technique for breast reconstruction in patients who are not good candidates for TRAM flaps. The ELD flap is technically simple yet capable of achieving excellent results. It is most useful for very obese patients and for patients older than 65 years. It can also be used successfully in patients who have very small breasts and therefore do not require transfer of much tissue volume. The ELD technique has the disadvantage of leaving a significant donor site scar, but many patients do not find that objectionable and believe that the donor site scar is more than compensated for by the technique’s simplicity and relatively short convalescence.
