Histopathologic diagnosis of salivary gland disorders

Abstract

Salivary gland tissue is not restricted to the three paired major salivary glands: the parotid, the submandibular, and the sublingual glands. Minor salivary glands, the so-called accessory salivary glands, are present throughout the oral cavity and the upper and lower respiratory tract. Since salivary gland tissue is so widespread, disorders of these glands can be found anywhere in the head and neck region, although some areas are more often involved than others. The only areas that are generally spared are the dorsum of the tongue and the anterior part of the hard palate and the gingiva. That is because the mucosa does not contain salivary gland tissue at these sites. Histopathologic diagnosis of a salivary gland disorder requires tissue for examination. This tissue specimen can be obtained by incisional or excisional biopsy. Biopsies can easily be taken from lesions involving the accessory glands, as these glands are situated superficially in the mucosal lining. Lesions of the major salivary glands are more deeply located; therefore, an initial diagnosis should be based on fine-needle aspiration biopsy and subsequent cytological examination.
