Physiological Study of the Lower Digestive Tract in Primary Constipation

Abstract

Since the classical study by Connell (1) it is a common believe that, in constipation, the major motor disorder consists in oversegmentation of the sigmoid colon providing a pressure barrier for the transit of stools. Although often confirmed by others (2–5), this opinion might be well disputable since a single digital examination of the rectum proves high incidence of stool retention in the rectum of constipated patients. Moreover, in a previous study of sigmoid motility in primary idiopathic constipation, we observed that sigmoid motor functions were normal in most of cases (6). Furthermore, frequent anorectal motility disorders were found, recently, in constipated patients (7–10). The present study was therefore undertaken to establish which motility disorder is primarily involved in chronicidiopathic constipation. To this aim, manometric studies of the sigmoid colon and of the anorectum were carried out; a compliance study of the rectum was added to the manometric investigation.
