Abstract

Unstable angina is a clinical term used to describe a number of syndromes of varying aetiologyl. The need to identify patients with unstable angina is obvious as it frequently leads to myocardial infarction and death2,3. The definition of unstable angina is angina pectoris that is increasing in either frequency, duration or intensity in the absence of myocardial necrosis. This definition includes rest angina, variant angina, and angina of recent onset in addition to increasing symptoms in patients whose angina was previously stable. Although by definition a patient who has stable angina and then gets one extra episode of angina per week may be said to have unstable angina, this is not what is meant in clinical practice. Similarly, by definition, every patient must go through an ‘unstable’ phase when their angina starts but if it only occurs on walking five miles it can hardly be said to be unstable. Thus the practical definition should include a modification, which requires a change in angina symptoms of significant severity to require hospital admission
