Zusammenfassung
Die 53-jährige Patientin wurde wegen Wesensveränderung, Apathie, Erbrechen und eines generalisierten tonisch-klonischen Anfalls aufgenommen. Bei der Aufnahme war die Patientin soporös und verlangsamt, sie klagte über holozephale Kopfschmerzen. Der neurologische Befund war normal, der Liquor unauffällig. Im CT des Kopfes und der CT-Angiographie zeigte sich eine Thrombose der tiefen Hirnvenen. Es wurde eine PTT-wirksame Heparinisierung begonnen, die zu keiner Besserung führte. Daher wurde im Rahmen eines individuellen Heilversuchs eine mechanische Thrombektomie unternommen, die zu einer inkompletten Rekanalisierung führte. Wegen der Entwicklung eines Hydrozephalus wurde eine lumbale Liquordrainage angelegt. Im Verlauf kam es zu einer Besserung der Vigilanz. Bei Verlegung bestand noch eine leichte psychomotorische Verlangsamung. Die Ursache der Hirnvenen- und Sinusthrombose konnte nicht geklärt werden.
