Zusammenfassung
Das leitliniengerechte präoperative Staging des Rektumkarzinoms umfasst neben der histologischen Sicherung des Tumors eine starre Rektoskopie mit Endosonographie, mindestens ein MRT des Beckens, die Bestimmung des karzinoembryonalen Antigens (CEA), eine vollständige Koloskopie sowie ein Röntgen-Thorax und eine Sonographie des Abdomens. Besteht der Verdacht auf Filialisierung oder bestehen unklare Befunde, so werden die letzten beiden Untersuchungen durch eine Thorax- und Abdomen-CT ergänzt. In der Detektion kleinerer Leberherde ist dabei das kontrastmittelverstärkte MRT der Abdomen-CT mit portalvenöser und arterieller Kontrastierung überlegen. Auch die Kontrastmittelsonographie erreicht in geübter Hand eine gute Sensitivität zur Detektion von Lebermetastasen.
