Andrologie in der interdisziplinären Reproduktionsmedizin

Zusammenfassung

Unerfüllter Kinderwunsch ist das Resultat von weiblichen und männlichen Infertilitätsfaktoren. Bei mindestens der Hälfte der betroffenen Paare ist mit Fertilitätsstörungen auf Seiten des Mannes zu rechnen, die auf verschiedenste Ursachen zurückzuführen sein können. Nach ihrer Lokalisation lassen sich Störungen der Hoden, der ableitenden Samenwege und akzessorischen Drüsen, der Samendeposition, des übergeordneten Hypothalamus-Hypophysen-Systems sowie Androgenrezeptor- und Enzymdefekte unterscheiden. Männliche Infertilität kann hierbei Endzustand sowohl angeborener als auch erworbener Erkrankungen sein; häufig liegen mehrere ätiopathogenetische Faktoren vor. Somit ist die Untersuchung des Mannes bei unerfülltem Kinderwunsch unverzichtbar, insbesondere auch vor einer assistierten Reproduktion. Bei nicht ausreichender gynäkologisch-andrologischer Kooperation besteht die Gefahr, dass auf Seiten des Mannes gesundheitsrelevante Störungen unerkannt bleiben und kausale Therapieoptionen nicht genutzt werden.
