Multimodale Therapieansätze bei der chirurgischen Therapie des Bronchialkarzinoms im Stadium III

Summary

In nonpretreated patients with tumor stage ‘limited disease,’ a combination of chemotherapy and radiotherapy leads to remission rates of 70%–95%. The mean survival time of patients is 12 months, in complete remissions 15–20 months. However, after 2 years, only 5%–10% of patients are free of relapse. In stage IV (metastatic disease) of non-small-cell lung cancer (NSCLC), chemotherapy is only palliative, with remission rates between 25%–45%. In stage III,new results with remission rates between 50%–70% suggest treatment with preoperative induction chemotherapy or new attempts with adjuvant chemotherapy, respectively.

Zusammenfassung

Im Stadium limited disease werden bei nicht-vorbehandelten Patienten durch die Kombination von Chemotherapie und Radiotherapie objektiv meßbare Remissionen in einer Häufigkeit von 70-95% erzielt. Die medianen Überlebensdauern der Patienten betragen 12 Monate, bei kompletten Remissionen 15-20 Monate. Nach 2 Jahren leben jedoch nur noch 5-10% der Patienten rezidivfrei. Bei den nicht-kleinz. Bronchialkarzinomen hat die Chemotherapie im Stadium IV mit Remissionsraten zwischen 25-45% ausschließlich palliativen Charakter. Neue Ergebnisse im Stadium III mit Remissionsraten zwischen 50-70% ergeben Ansätze für eine präoperative Induktionschemoth. bzw. einen erneuten Versuch der adjuvanten Chemotherapie.
