Zusammenfassung

Bisher liegen wenige durch adäquate Studien gesicherte Konzepte zum diagnostischen und therapeutischen Procedere beim Schlaganfall vor. Bei den intrazerebralen Blutungen (ICH) werden die Indikationen für neurochirurgische Interventionen kontrovers diskutiert, wobei das Spektrum von der Exstirpation aller raumfordernder Hämatome bis zur grundsätzlich konservativen Behandlung der Hirnblutungen reicht (3, 5). Noch uneinheitlicher erscheint das Management des akuten Hirninfarktes (1,2,4,6,8). Allseits laufen Studien zur Prüfung von Kalzium-Antagonisten, Hämodilution sowie mehrerer potentiell antiischämisch oder ‘zerebral protektiv’ wirkender Substanzen.
