Zusammenfassung
1993 beschrieb Dahlbäck bei Erwachsenen mit tiefen Beinvenenthrombosen die autosomal dominant vererbte Resistenz gegen aktiviertes Protein C [1]. Er beobachtete, daß bei diesen Patienten die Zugabe von aktiviertem Protein C (APC) die aktivierte partielle Thromboplastinzeit nicht in normalem Ausmaß verlängert (APC-Resistenz). 1994 zeigte derselbe Autor, daß hochgereinigter humaner Faktor V diesen Defekt in vitro korrigieren kann [2]. Im gleichen Jahr beschrieb Bertina bei 90% dieser Patienten eine Punktmutation im Gen des Faktor V (Nukleosidaustausch an der Positon 1691 G → A) [3]. Diese Mutation bewirkt, daß der aktivierte Faktor V bei erhaltenen prokoagulatorischen Eigenschaften durch aktiviertes Protein C verlangsamt inaktiviert wird mit der Folge eines Ungleichgewichtes der Hämostase im Sinne einer Zunahme der prokoagulatorischen Eigenschaften.
