Zusammenfassung
Atypische Umleitungen in Form des extraanatomischen Bypass kommen immer dann in Betracht, wenn ein besonderer Lokalbefund die In-situ-Korrektur oder die orthotope Bypassführung verbietet, oder wenn durch Verzicht auf die Eröffnung großer Körperhöhlen eine Verringerung der OP-Belastung erreicht werden soll. Ist die Indikation beim „besonderen Lokalbefund” durchweg klar, so ergibt sich beim „schlechten Allgemeinzustand” häufig ein variabler Ermessensentscheid.

Summary
Atypical bypass procedures using extraanatomic technique are to be considered (1) when certain local conditions do not permit in situ repair of a vessel or an orthotopic course of a graft and (2) when the avoidance of thoracotomy/laparotomy seems to promise a reduced surgical risk. While the indication given by “special local findings” normally will be a clear one, there is room for discussion in the so-called poor general condition patients.
