Ipsilaterale Femur- und Tibiafrakturen — Eine Indikation für ungebohrte Marknagelungen?

Summary

In a follow-up study, the clinical and radiological results of operative approaches to ipsilateral fractures of the femur and tibia (“floating knee”) were evaluated. There were comminuted C-type fractures according to the AO in 36% of the femur (30% of these open), and in 40 of the tibia (43 % of these open). The comparison of the osteosynthesis procedures plating, reamed and unreamed interlocking nailing and external fixation showed that the most favourable clinical results were obtained by simultaneous unreamed interlocking nailing of both the femur and the tibia. As a result of our study, this fracture combination proves to be an indication for simultaneous, unreamed interlocking nailing, especially in poly traumatized patients.

Zusammenfassung

44 Patienten wurden nach operativer Therapie einer ipsilateralen Femur- und Unterschenkelfraktur („floating knee“) klinisch und radiologisch persönlich nachuntersucht. Beim Femur lag zu 36 % und bei der Tibia zu 40% eine C-Trüm- merfraktur nach AO vor. 30 % der Femur- und 43 % der Tibiafrakturen waren offen. Der Vergleich der Osteosyntheseverfahren Platte, gebohrter und ungebohrter Verriegelungsmarknagel sowie Fixateur externe zeigte, daß die ungebohrte Marknagelung sowohl des Femurs als auch der Tibia die günstigsten klinischen Ergebnisse erbrachte. Diese Frakturenkombination stellt aufgrund unserer Erfahrungen eine Indikation für die simultane, ungebohrte Verriegelungsmarknagelung auch beim Polytraumatisierten dar.
