Zusammenfassung
Refluxsymptome, insbesondere das Leitsymptom Sodbrennen, sind der Mehrzahl der Bevölkerung bekannt: Etwa 18% haben im Laufe eines Jahres mit entsprechenden Beschwerden zu tun und lassen sich, ohne den Arzt aufzusuchen, in der Apotheke oder Drogerie mit rezeptfreien Medikamenten gegen Säurebeschwerden versorgen. Nach den Untersuchungen von Graham et al. [7] kann man davon ausgehen, daß bei einer Person, die öfter als 6mal pro Woche wegen Refluxbeschwerden zu einem Antazidum greifen muß, eine endoskopisch verifizierbare Refluxösophagitis vorliegt. Sontag [29] spricht, was die gastroösophageale Refluxkrankheit anlangt, von einem Eisbergkonzept, wobei die überwiegende Mehrzahl der Patienten nur geringe oder sporadische Symptome aufweist („telephone refluxers“). Einer Gallup-Umfrage zufolge sind dies 44% der erwachsenen Bevölkerung der USA. Bei den „office refluxers“, die wegen chronischer Symptome den Arzt aufsuchen, weisen 40–50% bei der Inspektion der Speiseröhre erosive oder ulzeröse Schleimhautdefekte auf, so daß von einer Prävalenz der Refluxösophagitis von 2–5% in der Bevölkerung auszugehen ist.
