Intraoperative Parathormonkinetik — Implikationen für das chirurgische Konzept / Intraoperative Parathyroid Hormone Analyses: Implications for the Surgical Concept

Summary

Between 1 July 1999 and 31 March 2001 seventy-five patients underwent parathyroid exploration with intraoperative PTH measurement. The reduction in detectable PTH levels of 90% of baseline 10 minutes after removal of an adenoma was shown in 73 cases. All 73 patients underwent successful parathyroidectomy and were cured for their hypercalcemia. In two cases with incomplete decrease of iPTH, their was a postoperative increase of the PTH concentration. Since 1/2000 in 66% we removed the adenoma without searching for any normal parathyroid glands if there was a 90% decrease in intact PTH concentration with a sensitivity, specifity and accuracy of 100%. The rapid intraoperative PTH analysis had excellent analytical performance if their is a decrease of 90% of baseline and predicted the success of parathyroid surgery in this series of patients. The iPTH testing is absolutely necessary in use of minimally invasive surgical parathyroidectomy. The use of iPTH monitoring reduce the operating time and minimize costs in therapy of patients with primary hyperthyroidism.

Zusammenfassung

Vom 1.7.99 bis 31.3.01 erfolgte bei 75 Patienten, bei denen eine Revision eines pHPT an unserer Klinik durchgeführt wurde, die intraoperative Parathormonbestimmung. Der geforderte Abfall um 90% gegenüber dem Ausgangswert nach Adenomentfernung konnte bei n=73 Patienten nachgewiesen werden. In allen Fällen kam es postoperativ zu einer Normalisierung des Serumcalciumspiegels. Bei zwei Patienten war der Abfall inkomplett, post-OP kam es hier wieder zu einem Anstieg des Parathormons. Seit 1/2000 erfolgte bei 66% der Patienten die Beendigung der Operation bei iPTH-Abfall um 90% ohne weitere Exploration und Probebiopsie eines normalen EPK nach EPK-Adenom-Extirpation bei einer Sensitivität, Spezifität und Accuracy von 100%. Die intraoperative PTH-Bestimmung zeigt bei einem Abfall des iPTH in den Normbereich und auf 1/10 des Ausgangswertes 10 Minuten nach Entfernung des Epithelkörperchenadenoms verläßlich die erfolgreiche Entfernung des hyperaktiven Nebenschilddrüsengewebes an. Sie stellt die Basis für den Einsatz minimal-invasiver Operationsmethoden dar und führt durch kürzere Operationszeiten zu einer Kostenreduktion.
