Epidermoid cyst with variable contents shown on CT and MRI

Summary

We report an epidermoid cyst which contained at least three kinds of material. One part of the lesion was of low density (25 HU) on CT, hypointense on T1-weighted images and hyperintense on T2-weighted images. This was a typical pearly tumour. Another part was of high density (96 HU) on CT and very low signal intensity on T2-weighted MR images. This contained viscous brown fluid, which was found microscopically to be haemosiderin. The third portion was calcification (400 HU) in the posterior wall. This epidermoid cyst arose from the third ventricle, a very rare location for such a lesion.
