Technik der Nieren- und Pankreastransplantation und der alleinigen Pankreastransplantation
Zusammenfassung
Die operative Technik und die Ergebnisse der Pankreastransplantation waren lange Zeit unbefriedigend. Seit Verwendung von Pankreasduodenaltransplantaten und Ableitung des exokrinen Pankreassekretes in die Harnblase und später in den Dünndarm ist die Pankreastransplantation aus chirurgischer Sicht sehr sicher geworden. Des Weiteren hat sie sich durch eine deutlich verbesserte Immunsuppression als Therapiemöglichkeit etabliert. Wesentlichen Einfluss auf den Erfolg einer Pankreastransplantation haben die Spenderkonditionierung, Organentnahme, Beurteilung des Transplantates und die Back-Table-Präparation, welche sich unmittelbar auf die postoperative Komplikationsrate auswirken.
