Laparoscopic fluorescence angiography with indocyanine green to control the perfusion of colorectal anastomoses intraoperatively

Abstract

The principle of fluorescence angiography using in docyanine green is known for a long time and was used especially by photographs and ophthalmologists for retinal diagnostic. We were the first who examined intraoperatively the perfusion of colorectal anastomoses by laparoscopic fluorescence angiography. In 2008 it was performed in 26 operations (6 right hemicolectomies, 2 transverse colon resections, 9 sigmoid resections and 9 deep anterior rectum resections) using a small bowel loop as reference perfusion. In all cases a correct perfusion of the anastomotic region could be shown. In cases of decreased perfusion a resection of this area could have been necessary. Laparoscopic fluorescence angiography is an easy and helpful method to detect a decreased perfusion intraoperatively. Further studies have to show, if the rate of anastomotic insufficiency can be lowered by using this new method.
