Zusammenfassung
Die Aufhebung oder Abschwächung hämodynamischer Nitratwirkungen unter Langzeittherapie wurde schon vor mehr als 100 Jahren beobachtet. Diese Beobachtung betraf besonders die Abschwächung des antihypertensiven Effektes von Nitroglycerin [21]. Die anhaltende antianginöse Wirksamkeit von Nitroglycerin wurde andererseits auch schon damals festgestellt und über vier Ärzte-Generationen immer wieder bestätigt [1, 4, 6, 8, 11, 13, 16]. In jüngster Zeit sind jedoch Zweifel an der anhaltenden antianginösen Nitratwirkung aufgetreten [12, 15, 21, 23, 24]. Häufig wurde ein Nachlassen hämodynamischer Effekte mit einer Aufhebung oder Abschwächung der antianginösen Wirkung gleichgesetzt [23]. Im folgenden soll gezeigt werden, daß unter chronischer Nitratbehandlung eine volle antianginöse Wirkung, trotz Aufhebung hämodynamischer Teileffekte, erhalten bleiben kann.
