Meniscal Allograft Transplantation Techniques

Abstract

Meniscal allograft transplantation (MAT) techniques continue to evolve. The common goal is to reproduce the function of the meniscus. To accomplish this goal, it is necessary to match the size of the meniscus allograft to patient’s original meniscus size, place the allograft root insertions at the original attachment sites, and create secure fixation. It is the authors’ preference to perform MAT via a bony attachment technique. Laboratory studies show that bony attachments are superior biomechanically to soft tissue only; therefore two bony attachment techniques will be illustrated in this chapter: bone bridge in channel and bone plug in the tunnel/socket. These techniques allow for reproducible results reported by multiple authors. The challenge for the next generation is meniscal extrusion: is it the key to chondroprotection?
